PER Help

I am working in an organization that is an approved employer. However, i’ve worked in two departments for the firm one is valuation and the other is audit. I want to know if i have to submit/fill up two separate forms for different departments as my line manager is different for both or one particular to a single manager. Also, if i do submit two forms, the manager for valuation department is not a qualified accountant who can sign it. Help me out with this guys.
